Medical Records & Billing – Emergicon

Lucas Fire-Rescue partners with Emergicon for EMS billing and medical records management. Please follow the appropriate instructions below to request records or address billing inquiries.

⚕️Medical Records Requests

For Patients:

  • Complete the Patient Records Request Form (PDF).

  • Submit the form with a copy of your driver’s license via:

    • 📧 Email: [email protected]

    • 📠 Fax: 800-608-9457

    • 📬 Mail: PO Box 180446, Dallas, TX 75218

  • Both the form and a valid photo ID are required.

For Attorneys & Law Offices:

  • Submit all record requests through ChartSwap.com.

  • Use the EMS agency (Lucas Fire-Rescue) as the listed provider.

For Special Use (Transplant, Crash Investigation, Trauma Registry, etc.):

  • Include patient name, date of service, and any supporting documents.

  • A records specialist will follow up.
