The purpose of this form is to facilitate the submission of a compliment or a complaint regarding a Lucas Fire-Rescue firefighter. If you would like to use this form to compliment, please complete the information below and we will ensure that the employee receives the information. You may also use this form to submit a complaint against public safety personnel and complaints must be submitted in writing and may not be anonymous. The City requires detailed information regarding the complaint so that it may be investigated. We take complaints against our firefighters very seriously and require as much detailed information as possible to help use guide the decision-making process.
